Lehneis Orthotics and Prosthetics Associates, Ltd. understands the tremendous burden that can be created by health care expenses. We will make every attempt to try and ease that burden, and we ask that our patients understand the complex and costly process that is involved in fabricating custom prosthetic and orthotic devices. Our staff will be happy to assist you in establishing a payment plan, helping to file insurance claims or discussing possible alternative avenues of legitimate care and reimbursement. Lehneis cannot begin fabrication of a device until insurance verification is completed or other financial arrangements are made. Lehneis will help obtain all necessary documentation required by insurance carriers and from your prescribing doctor when applicable.
Payment is not required for pre-authorized services.
Lehneis will provide patients with a paid, itemized bill for submission to the insurance carrier. Private and union insurance patients with no secondary insurance are required to pay their co-insurance and deductible when Lehneis accepts assignment of benefits. Beneficiaries with secondary insurance, other than Medicaid, are required to pay a one-half deposit and the balance upon delivery of the completed device when Lehneis does not accept assignment of benefits.
Currently, we participate in most Managed Care programs. Deductibles, coinsurance, and co-pays depend on your plan, and we will bill you at the time of visit. We cannot begin fabrication without a prescription, insurance authorization, and applicable patient payment.
Medicare recipients with no secondary insurance are required to pay their co-insurance and deductible when Lehneis accepts assignment of benefits. Medicare beneficiaries with secondary insurance, other than Medicaid, are required to pay a one-half deposit and the balance upon delivery of the completed device for non-assigned claims.
Medicaid recipients are normally not financially responsible for any services rendered by Lehneis as long as the recipient is deemed eligible. In certain cases, copayments or coinsurance may apply.
Lehneis will investigate, on your behalf, all insurance policy coverage's pertaining to orthotic and prosthetic services prior to your appointment after all applicable information is provided to us. If Lehneis is not a provider for your insurance carrier, we will try to obtain an out of network approval with your insurance carrier. If we are unable to secure an authorization or out-of-network approval due to your insurance carrier's policy, you will be advised, and asked to pay one-half of the cost of the device at your first visit, and the balance upon delivery. Our staff is always available to address any of your insurance and financial concerns.
